We require 24 hours notice for cancellation of appointments. Failure to provide sufficient notice may result in a cancellation fee. Repeated missed appointments without notice may result in the termination of services.

Payment is due at the time of service. We accept various forms of payment including cash, credit cards, and select insurance plans. It is the patient's responsibility to verify insurance coverage for our services.

5. Patient Responsibilities

Patients are responsible for:

• Following the treatment plan and home exercise program
• Asking questions about their care and treatment
• Respecting clinic staff and other patients
